Last Updated: 2 15 2004 Q: "I was treated with Lovenox during my pregnancy for a DVT. I now breast-feeding. My ob-gyn told me that I should stay on Lovenox for the 6 weeks postpartum because coumadin might appear in the breast milk." A: Coumadin warfarin ; does not cross over into the breast milk. Women who need anticoagulants in the postpartum period and want to breast-feed can be on Coumadin warfarin ; . I usually prefer Coumadin warfarin ; in the postpartum period, to avoid the risk of osteoporosis associated with Lovenox and other low molecular weight heparins. Warfarin Coumadin ; is not detectable in the breast milk of mothers who take warfarin coumadin ; . It is therefore safe to take in the breast-feeding woman. Since only a limited number of women on warfarin have been studied, these data can not exclude occasional passage of small amounts of warfarin into breast milk, but such doses are unlikely to cause bleeding in the newborn. However, it may be prudent to check coagulation studies in the newborn at risk for vitamin K deficiency, such as the premature born baby, before advising breast-feeding to mothers taking warfarin coumadin ; . There are very limited data in the medical literature on other coumarin drugs see Q A 26 ; , such as Phenprocoumon Marcumar, Falithrom, etc ; , or Acenocoumarol Sinthrome, Sintrom ; . It is therefore not known, whether they are also safe. Phenindione and Anisindione see Q A 26 ; appear in breast-milk and should therefore not be used by the nursing mother. References: 1. Ithout the protection of basic human rights, there can only be diminished mental health. Yet psychiatry routinely violates the human rights of those who seek its help. With this inherent contradiction in psychiatry itself, it has fallen to outside groups to protect those who are victimised by psychiatry's inhumanity. To that end, in 1969 CCHR members penned a Declaration of Human Rights for "mental patients and salbutamol, for instance, pregnancy.
But there have been posts here from others who said their insurance does pay for it, and still others who said their insurance company at first denied payment but after their doctor wrote them that the prescription was medically necessary did pay for it.

Mg123 of intrathecal morphine and repeated iv morphine by patient-controlled analgesia PCA ; pump for 24 hr results in appropriate analgesia, defined by VAS scores 3.17 So far - in approximately 6, 000 patients - the intrathecal dose of 0.1 mg morphine proved safe in that late respiratory depression did not occur. However, there continues to be a high incidence of other side effects such as urinary retention and itching.18 Also, the total iv PCA morphine dose is highly variable and ranges from 10 to 100 mg24 hr1. The present double-blinded randomized study was designed to test whether a prior exposure to the cyclooxygenase inhibitor ibuprofen would reduce preoperative and postoperative pain and whether it would augment morphine's potency. Methods and materials The Ethical Committee of our hospital approved the study and written informed consent was obtained from all patients. Fifty consecutive patients with coxarthrosis scheduled for primary elective total hip replacement surgery under intrathecal anesthesia were included. Exclusion criteria were those that exclude spinal anesthesia or the use of non-steroidal antiinflamatory drugs NSAID ; or opioids. Patients were allocated and randomized to two groups in a double-blind manner. All patients were pretreated during a two-week period before surgery: Group I with placebo drug, and Group II with ibuprofen 600 mg. Placebo and ibuprofen were prepared as look-alike tablets by the pharmacist, who was the only person aware of the type of pretreatment, and were given orally three times a day. The day of surgery all patients started with 15 mg movicox orally one hour preoperatively. This was continued for three days postoperatively. Prophylaxis against thromboembolism was started in all patients the evening before surgery with 3 mg acenocoumarol orally. On the day of surgery 2 mg acenocoumarol was given 24 hr after the initial dose. All patients were premedicated with 7.5 mg midazolam orally one hour before intrathecal anesthesia. Spinal anesthesia was induced by the administration of 20 mg bupivacaine plus 0.1 mg morphine dissolved in 4 mL isobaric solution ; . Adequate sedation was provided at the patient's request during the procedure: the anesthesiologist administered 1 mg midazolam at the minimum interval of five minutes until the patient indicated that the desired sedation was reached. Non-invasive blood pressure, heart rate ECG ; , SpO2, and respiratory rate were continuously monitored during anesthesia and in the intensive care unit during the first 24 hr after surgery and alfacalcidol.

Experimental, if not clinical evidence, suggests that drug resistance occurs as a result of spontaneous generation of resistant mutants with subsequent selection of drug-resistant cells under chemotherapeutic pressure. It may also be significant that antineoplastic drugs are themselves mutagenic, thus increasing the pool of potentially resistant mutants. Resistance is often seen in cancer cells following relapse after initial treatment and a period of remission. Drug resistant cancers, especially those that exhibit multi-drug resistance MDR ; , are much more refractory to treatment and are associated with poor prognosis.5 The keys to avoiding or decreasing the incidence of resistance are: 1 ; early treatment of cancer to avoid multiple resistant populations, and 2 ; the use of multiple agents with different mechanisms of action. It is postulated that drug resistance can occur at any step in antineoplastic drug action on the cell. These include entry into the cell, conversion and maintenance of activity and concentration within the cell, and ability to cause damage at the drug target. As most anti-neoplastic drugs rely on DNA damage and apoptosis for their action, DNA repair must not be efficient enough to counter their action and apoptotic mechanisms must be in place. Some key examples of resistance mechanisms and the drugs they affect are outlined in Table 1.

What conclusions about the function of the two auditory interneurones can be drawn from the measurements of their performance ? The alpha unit is clearly specialized for the reception of high-frequency sound. This has obvious uses. The genus stridulates by rubbing the hind femora on the elytra, but lacks the well-developed tooth or peg structure, as for example in the truxaline grasshoppers, which results in a peak of emission in the audible range. The sound produced is a high-frequency hiss, and oscilloscope examination confirms that most of the emission is ultrasonic. The animal lives in grass, and grass moved by the wind or other animals emits much high-frequency sound. Insectivorous predators such as shrews and bats emit ultrasound at high intensity; large-scale nocturnal flights are characteristic of many species of solitary grasshopper, and we have caught Gastrimargus at light at night. Haskell 1957 ; describes a take-off response by Schistocerca gregaria to the noise generated by aflyinglocust, and S. vaga responds similarly to the noise made by a vacuum cleaner. Haskell's recordings did not extend much beyond 10 kcyc. sec, but it seems probable that high frequencies are important in both cases. Popov 1965 ; performed ablations on the three morphological groups of sensilla in the ear of Locusta, and found that only those with a predominantly low-frequency less than 10 kcyc. sec. ; response gave rise to patterned summated potential, mirroring the characteristic amplitude modulation of the species song, in the recording from the tympanic nerve. The Group I sensilla, responding preferentially to higher frequencies greater than 10 kcyc. sec ; , did not; the author accepts that amplitude modulation is the most important parameter of song, and therefore suggests that the high-frequency sensilla are not important in song reception. The alpha neurone, which is driven mainly by high-frequency receptors, might by this argument be adapted for some function other than song reception. However, this argument contains too many assumptions about both song reception and the as yet unrecorded ear sensilla to be particularly attractive. The input arrangements to the alpha neurone seem to favour high sensitivity at the price of directionality. The unit is certainly very sensitive to at least some elements of a white-noise click, and we suspect that its most sensitive frequency is higher than our highest test frequency of 40 kcyc. sec. Amplitude changes are reasonably well encoded over some 30 dB of the response range. The unit's rather tonic response to medium length pulses would make it very suitable for signalling the length of such sounds. In a second paper Rowell & McKay, 1969 ; , we show that there is marked habituation to repetitive stimuli, but that at practical repetition frequencies the response never fails completely. We have much less information about the beta unit, but it is of great interest to see that it is clearly specialized for accurate directional discrimination, and that the feed-back mechanism by which this is achieved is functionally identical to that used by the tettigoniid grasshoppers Gampsocleis and Homorocoryphus Suga & Katsuki, 1961; McKay, 1968 ; . In other ways the tettigoniid interneurone differs considerably from the beta unit; for example, it is highly phasic in response, shows little habituation in the intact animal, gives a good low-frequency response and is very large in size McKay, 1968 but directionality is achieved in the same way and amantadine.

Parasitosis. Clinical management and treatment regimens were established, pathophysiologic studies were done, and epidemiologic information was gathered. One of the most complex challenges, however, was to determine the means of transmission of the parasite and its life cycle. First, attempts were made to determine possible reservoir hosts for adult and larval stages of the parasite. The Ilocano populations of Northern Luzon have unique dietary habits, unlike those of other Filipino populations. These eating habits were considered when searching for a source of infection. More than 150, 000 specimens of animal life were examined, and although adult Capillana worms were found, none were C. philippinensis. Capillaria larvae were also found, but species determination was not possible 15, for example, .

Sample Nearly 9000 unique patients are seen in the Family Medicine Clinic each year by family medicine faculty, residents, physician assistants, and behavioral therapists. Approximately 9.4% of these patients are diagnosed with depression. Subjects 18 years of age and older taking antidepressants for depression as diagnosed by a family medicine physician or psychiatrist were recruited from an outpatient Family Medicine Clinic at the UTMB at Galveston. Participants were excluded from the study if they discontinued taking the antidepressants because they were not currently suffering from depression or if the antidepressants were not being prescribed for depression. An effort was made to identify all patients who either received a new prescription for an antidepressant medication to treat depression or were currently taking an antidepressant for treatment of depression at the time of any clinic visit. All patients so identified were approached and asked to complete the Mood Disorder Questionnaire MDQ ; and a demographic and clinical history form on a completely voluntary basis. A subsequent sample of people who completed this battery and agreed to participate in an in-depth telephone interview underwent a modified Structured Clinical Interview based on the DSM IV SCID ; within several weeks of completing the MDQ. We attempted to interview all patients who agreed to the interview and who screened positive on the MDQ. We interviewed approximately an equal number of patients sampled from the patients who screened negative on the MDQ and who agreed to the telephone interview. Brian Threlfall is right to highlight the shortcomings of a "claims made" insurance policy under which indemnity cover will only apply if a claim is made while the policy is in force PJ, 3 April, p413 ; . National Pharmaceutical Association members -- and those they employ or engage -- will be reassured to learn that their policy is offered on a "claims occurring" basis through an NPA subsidiary: the Chemists' Defence Association. Under this policy, cover is maintained and claims will be met after the policy ceases to have effect. So members do not need to worry about claims arising after they have sold the business or retired. Consequently, they do not face the hassle, cost and worry associated with taking out the additional "run off " insurance referred to by John Murphy PJ, 1 May, p541 ; . It is our view that to offer anything less than a "claims occurring" policy would not be giving our members the level of service they are entitled to expect. It also gives out a valid and powerful quality assurance message to pharmacy's patients and customers. Mr Murphy seeks to imply that the cover provided by the NPA cannot be relied upon by employees because the NPA will seek to pass claims involving employees on to another body. This is not so. I wish to make it crystal clear that in settling a claim, we have never, nor will we ever, seek to reclaim any costs from an individual pharmacist or other member of pharmacy staff. Introduction Glucoamylases [1, 4--D-glucan glucohydrolase GA ; , EC 3.2.1.3] are a family of enzymes produced by fungi, yeast and bacteria that catalyze the cleavage of - 1, 4 ; glycosidic bonds in starch and related oligosaccharides to release -glucose from the substrate. GAs from Aspergillus awamori and Aspergillus niger have the same amino acid sequence, and they are extensively used in the food industry for hydrolyzing starch to produce highglucose syrup and ethanol. A thermostable form of GA is desirable since an elevated operating temperature will eliminate the chance of microbial contamination as well as drive the reaction at a higher rate. The process of GA thermoinactivation is dominated by formation of enzymes with incorrect conformation Munch and Tritsch, 1990 ; . Our previous work supports this hypothesis. The N182A, D257E, D293E and D293Q mutations eliminated.
