In a similar study, researchers examined use of complementary and alternative medicine CAM therapies ; among children with autism and found that 31.7 percent were currently using one or more CAM therapies, with 16.9 percent using a biological treatment with no basis in theory, 15.5 percent using a biological treatment with some basis in theory, 8.8 percent using a potentially harmful biological treatment, and 3.9 percent using a nonbiological treatment Levy et al., 2003 ; . Investigators from this team later reported treatment use among a larger sample of parents of children with autism, this time categorizing treatments according to their hypothesized mechanism: immune modulation 25.4 percent ; , gastrointestinal function 32.5 percent ; , neurotransmitter regulation 59.2 percent ; , and behavioral non-biological; 79.9 percent ; . Overall, this sample of children had tried an average of 6.2 different treatments SD 4.7 ; and were currently using an average of 3.6 SD 3.3; Mandell et al., 2005 ; . More recently, Green et al. 2006 ; collected data about treatment use through an online survey from parents of children with Asperger's syndrome AS ; , mild high functioning autism, or severe autism. Families were currently using an average of seven different treatments and had previously tried an average of eight. Parents of younger children and those with severe autism reported using more treatments than did parents of older children and parents of children with mild moderate ASD. The most commonly used interventions were those that focused on skills training e.g., speech therapy, ABA-based behavioral intervention however, among these most frequently used treatments, some were well validated with empirical evidence e.g., speech therapy ; and others were not e.g., sensory integration ; . More than half the sample indicated that their children were taking at least one medication. Collectively, these results highlight the fact that families are using a variety of treatments simultaneously for their children with ASD, some of which are chosen without supportive scientific evidence. It is important that professionals who work with this population understand the range of interventions that parents may be implementing so that they can assist families' choices of treatments likely to be most helpful to their children and systematically monitor their efficacy. This is especially true in cases where elimination diets and megadoses of select vitamins are being used, as there is no evidence yet supporting their efficacy, the health ramifications are questionable, and many children are not monitored by medical professionals during the course of these treatments Arnold, Hyman, Mooney, & Kirby, 2003; Levy & Hyman, 2003; Levy & Hyman, 2005 ; . However, receptor occupation by a neurotransmitter is only the first of many events in the neuronal signal transduction process. Post-receptor signal transduction events in the dopamine and serotonin systems frequently involve cycles dependent on activation of phospholipase PL ; A2 or PLC. There is recent evidence for two abnormalities of the PLA2 cycle in schizophrenia. Increased bioactivity of PLA2 and increased PLA2 levels have been observed both in blood and brain. Red cell and brain phospholipids in schizophrenia contain reduced amounts of arachidonic acid and docosahexaenoic acid, consistent with overactive PLA2. At the same time there is evidence of underactivity of fatty acid coeynzme A ligase FACL ; an enzyme which helps to terminate the PLA2 cycle. These phospholipid abnormalities have recently been supported by results of niacin skin testing and magnetic resonance imaging. Eicosapentaenoic acid EPA ; is able to inhibit PLA2 and activate FACL, so reversing the abnormalities in schizophrenia. Two open label studies and two randomised placebo-controlled trials in a total of around 90 patients have shown beneficial effects of EPA, apparently greater than those achieved with traditional neuroleptics. Two large multicentre trials of EPA are in progress in the USA and UK and will report their results by the middle of 2000. In in vivo proton magnetic resonance spectroscopy studies, neonatally lesioned rats show attenuated prefrontal cortical levels of N-acetylaspartate, a measure of neuronal integrity. Electrophysiological studies suggest changes in the function of prefrontal cortical neurons after stimulation of the ventral tegmental area. Right now, there is no single authoritative answer to this very important question. From anecdotal accounts in the press, by no means an adequate substitute for serious empirical study, there are three broad categories of drugs that frequently are abused: Opioids: Opioids are usually prescribed for their effective analgesic, or pain-relieving properties. Central Nervous System CNS ; Depressants: CNS depressants are substances that can slow normal brain function. Because of this property, some CNS depressants are useful in the treatment of anxiety and sleep disorders. Among the medications that are commonly prescribed for these purposes are: Barbiturates, such as Mebaral mephobarbital ; and Nembutal pentobarbital sodium ; which are used to treat anxiety, tension, and sleep disorders. On 11 June 2003 the Commissioner received a complaint from Ms A about the dental services she received from Dr B, oral surgeon. The complaint was summarised as follows: Dr B extracted Ms A's wisdom teeth on 30 October 2002. Between 30 October 2002 and 13 February 2003 Dr B did not provide dental services of an appropriate standard. In particular Dr B: did not identify a piece of bone lodged in Ms A's sinus as a result of the surgery failed to establish the cause of her ongoing symptoms of severe pain, constant dripping and smell, within a reasonable time frame and lasix.
The good news is that gastric cancer incidence rates are decreasing in Japan, which means that a smaller percentage of the population is being diagnosed with stomach cancer. The bad news is that the absolute number of people with gastric cancer is increasing due to the rapid aging of the Japanese population.1 However, there is more good news. In Japan, gastric cancer is usually diagnosed at an early stage, resulting in improving survival rates. In fact, although gastric cancer is five times more prevalent in Japan than in the West, mortality rates are lower in Japan.2 Stomach cancer, or gastric cancer, is the growth of cancer cells in the lining and wall of the stomach. Most stomach cancers start in the mucosa, the innermost layer of the stomach. About 90 to 95% of malignant tumors of the stomach are adenocarcinomas.3 Other types of stomach cancer include lymphomas, sarcomas and other rare types. Surgery is the most common treatment for stomach cancer. Sometimes, especially in advanced cases, surgery may be combined with chemotherapy or radiation.4 There is much debate regarding the cause of stomach cancer. It is thought that a combination of hereditary and environmental factors play a part.5 According to the National Cancer Institute in the United States, since 1930 the incidence of gastric cancer in the U.S. has decreased four-fold, to approximately seven cases per 100, 000 people.6 The reasons for this decrease are unknown but are believed to be related to improved food storage or dietary changes such as reduced salt intake. It is also believed that Japanese gastric cancer incidence rates are decreasing due to lifestyle and dietary changes, such as reduced salt use, increased consumption of fresh fruits and vegetables, and improved food storage.7 Hopefully, these changes, as well as an avoidance of smoking, will lead to both a decrease in gastric cancer incidence rates and a decrease in the absolute number of Japanese diagnosed with stomach cancer.
