Pharmacology: In Equasym XL, the capsules comprise both immediate- and extended-release beads such that 30% of the dose is provided by the immediate release component and 70% of the dose is provided by the extended-release component. Time course of action: In line with its different dosing profiles, Equasym XL has a shorter duration of action than Concerta XL approximately 8 h ; but a larger effect in the 45 h immediately after dosing given similar total daily doses [70]. As with Concerta XL there is variation between individuals in the time course of action. Clinical aspects: At the time of writing, Equasym XL is only available in the UK. The drug is administered once daily in the morning, before breakfast. Our recommended starting dose is 10 mg once daily. The company's advice is that dosage may be adjusted in weekly 20 mg increments to a maximum of 60 mg day; our experience is in line with this, but there may be a limited number of cases who benefit from higher doses and the maximum for them is 2 mg kg day; or 100 mg total daily dose--whichever is the smaller and acetylsalicylic.

As discussed in the previous seminar, the goal of behavior therapy is get a relaxed and calm response in the presence of the stimulus response substitution ; and to achieve a positive association with the stimulus rather than one of fear counterconditioning ; . This requires first identifying all stimuli that might evoke fear to determine what will be the target of response substitution and counterconditioning and to prevent from the outset any further exposure that might cause injury or further increase the fear. Only when the owner can successfully and immediately control and calm the pet in the absence of the fear evoking stimuli, is it practical to begin exposure exercises, using rewards that are strong enough to achieve the desired response in the presence of a stimulus that is sufficiently mild desensitization ; . When the pet is anxious or fearful, the sympathetic nervous system is stimulated and tracts to the adrenal medulla lead to the release of noradrenaline and adrenaline, which primes the body for active defense or retreat. Endorphins are released and sensitivity to pain falls. Fear and anxiety also affect the pituitary adrenal axis, leading to an increase in cortisol release. When the level of anxiety and arousal is high the pet will react automatically reflexively ; to the threat so that new learning cannot occur. In addition, pets that are highly aroused are in an emotional state where behavior modification is impractical. Therefore, behavior management products such as head halters can provide for more immediate control and safety and may help to calm the pet. Drugs may also aid in reducing the fear and anxiety so that the pet can better focus on the training and behavior modification. In addition, if the pet is excessively fearful or anxious, drugs may be in the pet's best interest. Physical control devices A head halter serves as a tool to communicate the owner's intentions by prompting the dog to achieve the desired response. In addition, by controlling the muzzle and mouth, biting, barking and lunging can be more safely and effectively prevented. Although animals as large as horses are commonly managed with devices that control the head and muzzle, most dog owners continue to try and control dogs with neck restraint often with limited success ; . Neck devices pull against the dog's trachea and allow the dog to use its legs and neck muscles to propel itself forward. Recent studies also indicate that pulling against a neck collar can lead to increased intraocular pressure.1 When unsuccessful some owners or trainers then escalate to choke, prong or shock devices which are counterproductive in the treatment of fear aggression since they act to deter undesirable behavior rather than to calm, control, and achieve desirable responses. In one study, the use of head halters in puppy classes was associated with higher retention while no such correlation was found with the use of shock, choke or prong collars.2 With the use of a head control device, "where the nose goes the body follows". Some head halters control the dog from behind the head, which can be a useful means of controlling pulling e.g. Canny Collar or Easy Walker ; . Similarly a number of new body harnesses which attach at the front of the chest are useful at limiting pulling. However, the use of head halters in which the leash attaches under the chin e.g. Gentle Leader, Halti or Snoot Loop ; provide control of the muzzle, which is of particular importance in dogs with aggression. This is because these head halters can be used to a ; reorient the head to focus on the owner and away from the stimulus to be able to calm or reward the pet by their actions and responses b ; prompt the dog to exhibit desirable responses such as sit focus or quiet c ; help the owner to gradually progress to longer and more relaxed responses downsettle, sit-focus ; d ; serve as a means of reinforcing desirable responses and insuring proper timing with an immediate release of tension negative reinforcement ; followed by positive reinforcement if the pet maintains the desirable behavior e ; insure greater safety by reorienting the pet away from the stimulus with a gentle pull assuming the owner is maintaining short slack ; and by closing the mouth if necessary and f ; providing a faster and safer means of calming the pet so that exposures exercises can be implemented. The Gentle LeaderTM has the added advantage in that it can be fitted to remain on the head i.e. a head collar ; even when the owner is not holding the leash. Thus the head halter can help fulfill all parts of the behavior program by preventing further injury and exposure, training the dog to settle and calm; and as an aid in implementing safe and effective exposure training. Training with a head halter One of the most effective means of gaining control and insuring that the pet quickly learns each command is to use a leash and head halter. Proper fitting and usage instructions are essential to achieve maximal success.

Thus, it is currently assumed that the mycobacterial enzyme possesses both nicotinamidase and PZase activities. This assumption is, however, in apparent conflict with the enzyme activities shown in Table 1. Whilst all the strains studied exhibited a nicotinamidase activity located in various cell envelope compartments Raynaud et al., 1998 ; this work ; only M. tuberculosis and M. smegmatis showed a detectable PZase activity. In addition, whilst nicotinamidase activity was found to be extracellular and\or surfaceexposed in both M. tuberculosis and M. bovis BCG Raynaud et al., 1998 ; , PZase activity was neither detectable in the outermost capsular components of M. tuberculosis nor in whole cells and the bacterial lysate of M. bovis BCG. The failure to correlate nicotinamidase and PZase activities could be attributable to different sensitivities of the tests used. However, this hypothesis is unlikely since we used 4 mM concentrations of nicotinamide and PZA, i.e. at least tenfold the highest Km determined for the two amides in mycobacteria Boshoff & Mizrahi, 1998 using this optimal condition, Tarnok & Rohrscheidt 1976 ; also found both nicotinamidase$ negative\PZase-positive and nicotinamidase-positive\ PZase-negative strains among the mycobacteria they examined, demonstrating that the failure to detect one of the enzyme activities was not due to the lack of sensitivity of the tests used. Thus, our data may be explained by the occurrence of more than one amidase able to hydrolyse nicotinamide in mycobacteria, one of which can also hydrolyse PZA. This hypothesis is also supported by several earlier observations, among them the fact that M. kansasii can hydrolyse nicotinamide but not PZA Tacquet et al., 1967 ; whereas ` Mycobacterium thamnopheos ' hydrolyses PZA but not nicotinamide Bonicke, 1960 ; and ` Mycobacterium balnei ' will grow $ on PZA as nitrogen source but not on nicotinamide Tsukamura & Tsukamura, 1966 ; . A parallel of the proposed phenomenon would be the mycobacterial catalases : different species possess one or more catalases Bartholomew, 1968 ; Que! mard et al., 1995 ; Wayne & Diaz, 1988 ; and one of these has an additional peroxidase activity.
